Reports section on the company record is the company specific reports. The reports are categorized as Demographic reports, Benefits/Payroll reports and Carrier forms.
Demographic Reports
Demographic Census
This Demographic Census report will provide employee census data that can be downloaded. To get census report, go to:
Companies → Select Company → Reports → Demographic Census
User filter to get appropriate census information. User can get census data with or without dependent information. Use “Include terminated” checkbox for terminated employees. Download census report in csv file format.
New Hires
This report provides a list of new employees hired within a specific time period. This is useful when you want to track employee enrollment, determine benefits eligibility, and so on.
Use the filters to retrieve the new hires’ data. Make sure all your company details such as location, organization structure, or pay groups are set up properly. If you haven’t configured these, you may not be able to select these options when generating your report. Click Apply to generate the report. You can download this report as a comma-separated values (csv) file.
Terminations
This report provides a list of employees terminated within a specific time period. This is useful to determine or track the continuation or cessation of benefits.
Use the filters to retrieve data on terminated employees. Click Apply to generate the report.
Demographic Changes
Report about employee and dependent profile info that was changed between X and X date (similar to audit log) is available.
Report Format:
- Employees and dependents are listed on separate rows.
- Dependents tied to employees via the employee’s SSN (all dependent rows should also include employee’s SSN)
Filter options:
- From Date and To Date (all changes that were processed in the system on and between given dates) - this is a required field
- Office Locations
- Groups (ex: Department)
- Included dependents

Enrollments
Employee’s enrollment election with coverage details can be retrieved by Enrollment reports.
Payroll Deductions
Report showing employee payroll deductions per product/plan are listed on separate rows. Filter options:
- Payroll Date - All employee deductions as of X date (required)
- Office Locations
- Groups (ex: Department)
Benefit Election Changes
Report of employee and dependent benefit election changes (new enrollments, ended enrollments, changed enrollments, etc.) is available. This report provides information about the following types of election changes:
- Plan
- Coverage Level
- Result of adding or dropping dependents from coverage
- Employee Cost
- Coverage Ended
- Dependents added/dropped
- Not all scenarios of adding or dropping dependents = change in coverage level. Example: Employee already enrolled in family coverage (has 3 CH enrolled) then has QLE due to have a baby and another child is added to coverage. Wouldn’t necessarily result in change to coverage level or cost, but change needs to be submitted to carrier.
Report Format:
- All products that employee is enrolled in will be listed on separate rows
- Employees and enrolled dependents are also listed on separate rows
- Dependents tied to employees via the employee’s SSN (all dependent rows should also include employee’s SSN)
Filter options:
- From Date and To Date (all changes that were processed in the system on and between given dates) - this is a required field
- Carrier
- Product
- Enrollment Type
- New Hire Enrollment
- Open Enrollment
- Newly Eligible Enrollment
- Qualified Life Event Enrollment
- Rehire Enrollment ? not in V1

- Include declines - If checked, includes employees election waivers

Payroll Deduction Changes
Report of employee payroll deduction changes.
Report Format:
- Employee’s deductions per product/plan are listed on separate rows
Filter options:
- From Date and To Date (all changes that were processed in the system on and between given dates) - this is a required field
- Office Locations
- Groups (ex: Department)
Passive Enrollment Audit
This report displays the issues that occurred during the Open Enrollment period.
Employee Enrollment Participation Rates
This report shows the percentage of employees that enroll (vs. decline) coverage per benefit (product) type. This helps the employer understand which benefits most of their employees elect, vs. which benefits most employees decline. Can help decide whether or not to renew specific benefits from month to month to accommodate mid year changes.
Example: Total number of eligible employees = 100
Out of those 100 eligible employees, 60 employees are enrolled Medical. So, Medical → 60%. Other 40% declined the enrollment.
Employer Total Monthly Cost
This report shows the employer’s total monthly cost (contribution amount) for all benefits offered to employees.
The total amount displayed = the total employer contribution amount for all active enrollments in all active plans as of the given month.
Scheduled Benefit/Rate Changes
This report provides a list of employees and their dependents who have scheduled future changes in benefit amounts for Life, AD&D, STD, LTD, Accident, Cancer, and Hospital Indemnity plans. The changes in the benefit amount may be because of mid-year birthdays that result in changes in age brackets applicable for benefits, or, reduction in benefits.
Use the following filters and click Apply to generate your report:
- Carrier/Provider: Select one or more providers. All active providers or carriers associated with your company are listed here.
- Product Type: Select one or more product types. All active product types of your company are listed here.
Scheduled Drop Over-Age Dependent Coverage
This report provides a list of employee dependents who have reached maximum eligible age for plans and their scheduled coverage end date based on the overage criteria (defined in the eligibility rules under each plan).
Use the following filters and click Apply to generate your report:
- Carrier/Provider: Select one or more providers. All active providers or carriers associated with your company are listed here.
- Product Type: Select one or more product types. All active product types of your company are listed here.
Open Enrollment Elections
This report provides a list of all OE selections that employees have made and changes or waivers to them if any. This data is sent to the carriers or providers so that they can track employee plans, waivers, or changes. Admins must send OE selection data to the carriers before the OE end date so that these plans remain active. This report is especially useful when you don’t have automated file feeds or connections with carriers and also enables you to track historical data for prior OE periods.
Select an open enrollment period to generate the report.
The report includes:
- Employee and dependent details such as name, address, SSN and so on
- Plan name and benefit amount
- Enrollment date
- Employee and company cost
- Reason for waivers
COBRA Eligibility
Employees and their enrolled dependents are entitled to continuous coverage under a group health plan called COBRA, even when their company benefits end due to certain circumstances such as employment termination, reduction in hours, death of employee, and so on.
Use the following filters and click Apply to generate your report:
- Eligible on or before: Select a date from the calendar.
- Office Locations: Select one or more locations if applicable.
- Other elements of your organizational structure (as they have been defined in the system): For example, Department name, Groups, Division, and so on. Select one or more as applicable.
Self-Bill Carrier Summary
Employees can do self-billing for some group plans such as Life/AD&D/STD/LTD. In this case, each month, you pay the carrier based on the total number of employees or dependents enrolled in these plans. This report is typically generated for the previous month. For example, in October, you generate the report for September.
Select the carrier and the date for which you want the report. All active plans during the selected period are retrieved.
Report format:
- Plan name or type
- Total number of employees enrolled
- Total cost of the plan
- Total premium paid for the plan
Self-Bill Carrier Detailed
This report is the same as the Self-Bill Carrier Summary report, but with more details.
Select the carrier and the date for which you want the report. All active plans during the selected period are retrieved.
Report format:
- Employee details and organizational structure
- Group Plan name or type
- Total cost of group Life/AD&D/Disability cost
- Total premium for each group plan
- Employee dependent cost and premium
Total Compensation Statements
This report provides details of an employee’s total compensation package that includes employee’s basic annual pay plus the company’s contribution towards the benefits selected by them.
To generate the report, first select a date and then select the appropriate filters.
Filters:
- Office locations
- Other elements of your organizational structure such as departments, divisions, or groups
Note that the filter options depend on how you have configured your organizational structure elements in the system.

Carrier Forms
Enrollments
Use this report to send all employee plan enrollments to carriers or providers in bulk. You can download this form as a .zip file. The .zip file includes enrollments in a PDF format categorized by the carrier or provider. For example, ABC_Company_Enrollment_Forms.zip will include Blueshield California.pdf, Delta Dental Insturance.pdf, Principal Financial Services.pdf and so on.

Enrollment Changes
Use this report to send enrollment changes done by employees to carriers or providers in bulk. You can download this form as a .zip file. The .zip file includes enrollment changes in a PDF format categorized by the carrier or provider.
